    Background
    Bromodomain-containing protein 4 (BRD4) is a member of the BET (bromodomains and extra-terminal) family of chromatin reader proteins, which also includes BRD2, BRD3, and BRDT, and is defined by the presence of two tandem bromodomains (BD1 and BD2) and an ET domain. BRD4 contains an N-terminal region with BD1 and BD2, each consisting of four α-helices connected by ZA and BC loops that specifically recognize acetylated lysines on histones H3 and H4, as well as conserved motifs for DNA binding, a basic interaction domain, a three-helix ET domain (amino acids 600–678), a SEED domain, and a C-terminal motif (CTM, aa 1325–1362) for mediator complex interactions. BRD4 binds acetylated chromatin throughout the cell cycle, including mitosis, where it acts as a bookmark for rapid gene reactivation after cell division. It recruits the pTEFb complex through BD2 and CTM to phosphorylate Ser2 on the CTD of RNA polymerase II, promoting transcription elongation, and scaffolds the super-elongation and PAFc complexes, particularly in MLL-fusion leukemias. BRD4 regulates inflammatory gene expression (e.g., via NF-κB/RelA), Myc-driven oncogenes, and survival genes like Bcl-2 and CDK6 in cancers such as Burkitt's lymphoma and NUT midline carcinoma, where BET inhibitors can displace BRD4 from chromatin to induce apoptosis and exert antitumor effects.
