    Background
    Caspase-1 (interleukin-1β converting enzyme, ICE), classified as an inflammatory cysteine protease of the caspase family alongside caspases-4, -5, -11, and -12, is synthesized as an inactive 45 kDa proenzyme essential for innate immune responses. It features an N-terminal caspase recruitment domain (CARD) for inflammasome assembly, a large p20 subunit containing the catalytic cysteine residue, and a small p10 subunit, which together form an active heterotetramer (p20)₂(p10)₂ through proximity-induced dimerization and autoproteolytic cleavage. Caspase-1 activates within multiprotein inflammasomes (NLRP3, AIM2, NLRC4/ASC) triggered by pathogen-associated molecular patterns or damage signals, specifically cleaving pro-IL-1β and pro-IL-18 into mature proinflammatory cytokines that drive fever, inflammation, and adaptive immunity, while processing gasdermin D to execute pyroptosis via N-terminal pore formation, causing lytic cell death and IL-1 release. Caspase-1 deficiency selectively impairs cytokine maturation and confers resistance to endotoxic shock without affecting apoptosis.
