    Background
    Syntaxin-4 (STX4) is a t-SNARE protein of the SNARE superfamily that plays a pivotal role in intracellular vesicle trafficking and regulated membrane fusion across various cell types. Syntaxin-4 has a long cytoplasmic N-terminal region with two coiled-coil domains, the Hab domain (involved in auto-inhibition) and the SNARE motif, a single C-terminal transmembrane domain anchoring it to the plasma membrane, and a short extracellular C-terminus. The Hab domain can fold back onto the SNARE motif in a “closed” conformation stabilized by Munc18c binding, with key N-peptide residues mediating this interaction. Syntaxin-4 forms trans-SNARE complexes with SNAP23/25 and VAMP2/7/8 to mediate membrane fusion, thereby regulating processes such as GLUT4 translocation in muscle and adipose tissue for insulin-stimulated glucose uptake, insulin granule exocytosis in β-cells, AMPA receptor trafficking in neurons, lytic granule release in cytotoxic T cells, and hepatitis C virus (HCV) particle secretion. Its activity cycles between closed (Munc18c-bound, inactive) and open (SNARE-competent) states, transitions triggered by Rab GTPases and phosphorylation events (e.g., by PKC/PKA at N-terminal sites), and integrates with PI3K/Akt and Ca²⁺ signaling pathways. Dysregulation of Syntaxin-4 is linked to diabetes (via impaired GLUT4 trafficking), neurodegeneration, and viral pathogenesis.
