Background
TIM-3 (T cell immunoglobulin and mucin-domain containing-3) is a transmembrane glycoprotein immune checkpoint receptor expressed on T cells, NK cells, dendritic cells, and macrophages. TIM-3 contains an extracellular IgV domain for ligand binding, a mucin-like domain with extensive O-linked glycosylation, a transmembrane segment, and a cytoplasmic tail with five conserved tyrosine residues serving as signaling hubs. As an inhibitory regulator, TIM-3 suppresses Th1 immune responses and induces T cell exhaustion in chronic infection and cancer by binding ligands such as galectin-9, CEACAM-1, and HMGB1/phosphatidylserine complexes. Ligand engagement leads to tyrosine phosphorylation, Fyn kinase recruitment, Bat3 displacement, and subsequent inhibition of TCR signaling, NF-κB activation, and cytokine production (e.g., IFN-γ, TNF-α). TIM-3 also restrains innate immunity by impairing nucleic acid sensing and cross-presentation in dendritic cells. Co-expression of TIM-3 with PD-1 marks highly dysfunctional T cells in tumors, and therapeutic TIM-3 blockade can restore anti-tumor immunity, especially in combination with other checkpoint inhibitors.
